Meta reviews every template and can reject one. The reasons are usually straightforward to fix.
The template is in the wrong category, for example a promotion submitted as utility. Variables have no sample values, so the reviewer cannot judge it. The content has a policy issue, like a misleading claim or prohibited content. Or formatting and placeholders are broken.
Most rejections come from sending marketing content under the utility category. Match the category to the actual content and the approval usually clears.
